Retatrutide and Keto: What the Trials Actually Tested

Neither the phase 2 trial nor TRIUMPH-2 tested a ketogenic diet with retatrutide. Compare their populations, measured outcomes and diet-related limits.

Retatrutide is investigational and is not FDA-approved. FDA states that it cannot be used in compounding under federal law. FormBlends does not prescribe, compound or sell retatrutide.

Direct answer: The published retatrutide trials do not establish whether a ketogenic diet improves results, is compatible with treatment, or changes side effects. They did not randomize participants to keto or compare named diets.

What the human studies actually compared

StudyParticipants and durationDiet comparisonWhat the result can answer
Phase 2 obesity trial, 2023; PMID 37366315338 adults with overweight or obesity; treatment through 48 weeksNo keto arm or randomized comparison of named dietsCompares retatrutide dose groups with placebo for the outcomes reported by the trial; it cannot identify a best diet pairing.
TRIUMPH-2, phase 3; PMID 428103721,152 adults with type 2 diabetes and overweight or obesity; 80 weeksNo ketogenic-diet intervention or keto-versus-other-diet comparisonReports retatrutide and placebo outcomes in this population; it does not estimate a keto add-on effect.

Why weight-loss results do not answer the keto question

A trial showing weight change while participants receive the study's background lifestyle instructions does not show that keto caused, improved or reduced that result. The retatrutide studies above compare drug assignments, not ketogenic diets. A diet-specific claim would need a study that measures the diet exposure and compares relevant outcomes between otherwise similar groups.

Questions the published trials leave open

  • They do not show whether keto produces more weight loss with retatrutide than another diet.
  • They do not establish a specific carbohydrate target for people taking retatrutide.
  • They do not compare gastrointestinal symptoms across ketogenic and non-ketogenic diets.
  • They do not quantify an interaction or compatibility effect between retatrutide and a ketogenic diet.

These are evidence gaps, not findings that keto is superior, inferior or incompatible. The available weight and glycemic results cannot be relabeled as a diet comparison.
