Who prescribes, who makes your medication, and how to check both
Prescriptions come from clinicians of Recess MD LLC, an independent medical group. Medication is dispensed by state-licensed 503A compounding pharmacies. Both are separate from Form Blends LLC. This page names them, explains how to verify a licence yourself, and tells you how to get the test results for your lot.

Who prescribes
Prescriptions are written by independent, state-licensed clinicians (MD, DO, NP or PA) of Recess MD LLC, a medical group separate from Form Blends LLC; you consent to telehealth care from Recess MD LLC providers during intake.
You can read the consent document before you start: telehealth consent. The clinician must be licensed in your state. Where clinically needed, or where your state requires a synchronous encounter, the clinician may ask for a video or phone visit before prescribing. Prescriptions are patient-specific.
Verify a clinician: The prescribing clinician's name and licence number appear on your prescription and your visit summary. Look them up on your state medical board; every board is linked at formblendsdirectory.com. State medical boards.
What the clinician reviews before prescribing
- Medical history, current medications and allergies
- Pregnancy status
- Weight and height, used to calculate BMI
- Contraindications, including a personal or family history of medullary thyroid carcinoma or MEN 2, a history of pancreatitis, and current GLP-1 use
- Identity verification
- Any labs or records you upload
The clinician may decline, request more information, or request a video or phone visit.
Follow-up
- A check-in is requested before dose changes.
- You can message your treating clinician through the patient portal at any time.
- We recommend seeing a primary care clinician for labs at least once a year.
Urgent symptoms: call 911 or go to urgent care. The portal is not monitored for emergencies.

What 503A means for your medication
- Your prescription is patient-specific: it is compounded after the clinician writes it, not pulled from a pre-made stock.
- It is prepared under USP 795 (non-sterile), USP 797 (sterile) and USP 800 (hazardous drugs) standards.
- A beyond-use date is assigned per the USP 797 category and printed on the label.
- Batch potency and sterility testing is available as a certificate of analysis on request.
The price does not change with dose: it is the same at 2.5 mg or 15 mg. Your clinician sets the dose. Shipping is included; lab work is not.
Zepbound, Mounjaro, Wegovy and Ozempic are FDA-approved products made by Lilly and Novo Nordisk with FDA-reviewed manufacturing. Compounded semaglutide and compounded tirzepatide are not FDA approved, are not generic versions of those products, and are not interchangeable with them. How to verify a pharmacy licence yourself
- Find the pharmacy name on your prescription label or shipping paperwork.
- Open the board of pharmacy licence lookup for the state where the pharmacy is located. All 50 state boards are linked at formblendscompounding.com.
- Search by pharmacy name or licence number. Confirm the licence is active and check for any discipline on record.
- Pharmacies shipping into your state usually also hold a non-resident pharmacy licence from your state's board. You can check that one too.

What a certificate of analysis shows
A certificate of analysis (COA) is the lab report for a specific lot. For a sterile injectable it typically reports potency (how much active ingredient is present against the labelled amount) and sterility, along with the lot number, the test dates and the laboratory that ran the tests. It is issued per lot, so the COA that matters is the one for the lot number printed on your vial.
You can request your lot's COA (potency and sterility results) and the pharmacy's state licence number from support at any time.
Shipping and storage
Orders ship after the clinician approves and the pharmacy compounds, typically 3 to 7 business days. Medication ships cold-chain with ice packs and you get tracking by email. The beyond-use date is printed on the label; use the vial before that date and store it as the label and the pharmacy's insert instruct. If a shipment arrives warm, damaged or incorrect, report it within 7 days with a photo and it is replaced at no charge. Details are on the returns and refunds page.

Can I see the certificate of analysis for my vial?
Yes. Email support@formblends.com with your order number and the lot number from your label. We request the lot's certificate of analysis from the dispensing pharmacy and send it to you.
Can I get the pharmacy's state licence number?
Yes. Ask support for the licence number of the pharmacy that dispensed your order, then look it up on that state's board of pharmacy site.
Are compounded GLP-1 medications FDA approved?
No. Compounded drugs are not FDA approved, are not generic versions of the brands, and are not interchangeable with Zepbound, Mounjaro, Wegovy or Ozempic.
