Where FormBlends patients live
As of September 5, 2026, FormBlends has dispensed compounded prescriptions to patients in 36 states and territories. That count comes from the last 165 shipped orders, not from a marketing map.
Availability in any state depends on three things at once: the reviewing provider must be licensed in your state, the dispensing pharmacy must be licensed to ship there, and your state's telehealth rules must allow the visit type used. The assessment checks all three for your address before any charge. If we cannot serve you, you are told at that point and nothing is billed.

What "dispensed to patients here" means
At least one FormBlends patient with a shipping address in that state received a compounded prescription in the last 165 shipped orders. It does not mean every patient in that state will be eligible. Eligibility is a clinical decision made by the independent provider after reviewing your intake.
What "check availability at assessment" means
We have not shipped to that state in the recent order sample. It may still be available. The assessment asks for your address first and confirms whether a licensed provider and a licensed pharmacy can serve you there. Some states require a video or phone visit before a prescription; if yours does, the provider will schedule one.
Who prescribes
Prescriptions are written by independent, state-licensed clinicians (MD, DO, NP or PA) of Recess MD LLC, a medical group separate from Form Blends LLC; you consent to telehealth care from Recess MD LLC providers during intake. Read the telehealth consent.
Verify a clinician: The prescribing clinician's name and licence number appear on your prescription and your visit summary. Look them up on your state medical board; every board is linked at formblendsdirectory.com. State medical boards.
