These are the prices you will see at checkout. No membership fee, no consultation fee, no shipping charge on top. If the provider does not prescribe, you are refunded.
Every plan is a one-time upfront charge for the plan length. Nothing renews unless you choose Subscribe & Save. The introductory price applies to the first month of any plan: $99 for semaglutide, $149 for tirzepatide. Subscribe & Save renewals bill the plan price minus a flat $25 subscriber credit. The price does not change with dose: it is the same at 2.5 mg or 15 mg. Your clinician sets the dose. Shipping is included; lab work is not.

Each plan is charged once, upfront, for its length. Your first month is billed at the introductory price. If you choose Subscribe & Save, the plan renews at the plan price minus a $25 subscriber credit, on the schedule of the plan you picked.
Cancel any time before the next billing date shown in your confirmation email by emailing support@formblends.com. Cancellation stops future charges and shipments and takes effect for the next cycle.
What is not included

- Care outside the program, such as visits with your own doctor or lab tests you arrange elsewhere, is not covered by the price
