Eligibility Requirements
- Enrolled in a Medicare Part D prescription drug plan
- BMI of 30 or greater (obesity), or BMI of 27 or greater with at least one weight-related comorbidity (type 2 diabetes, hypertension, dyslipidemia, or obstructive sleep apnea)
- Must have a prescribing physician who participates in Medicare
- Beneficiary must not be currently receiving the same medication through another federal program
