What does this video actually claim?
@tarzanofcali claims his penis shrunk due to low testosterone, using the euphemistic "PP ShrUNk" in his caption. He connects this to delayed puberty and positions testosterone therapy as the solution for "the boys."
The video plays into common fears about testosterone deficiency causing genital atrophy. It's targeting young men concerned about masculinity and sexual development, using gym culture hashtags to reach that audience.
The creator doesn't specify whether he's talking about flaccid size, erect size, or testicular atrophy. This vagueness makes his claim harder to evaluate scientifically.
Does low testosterone actually shrink your penis?
Adult penis size doesn't change with testosterone levels once puberty is complete. The Endocrine Society's 2018 guidelines on testosterone therapy don't list penis shrinkage as a symptom of hypogonadism in adult men.
What can shrink are the testicles. The Massachusetts Male Aging Study found testicular volume decreases with age and lower testosterone, but this affects the testes, not the penis itself.
During puberty, testosterone does drive penile growth. Boys with delayed puberty may have smaller adult penis size if untreated, according to research by Mauras et al. in the Journal of Clinical Endocrinology & Metabolism (2001).
What about testicular atrophy?
This is where the creator might have a point, though he's confused about the anatomy. Testosterone deficiency can cause testicular shrinkage in adult men.
The European Association of Urology found that men with hypogonadism had average testicular volumes of 12-15ml compared to 20-25ml in healthy controls. That's a real and measurable difference.
Interestingly, testosterone replacement therapy can actually worsen testicular atrophy by suppressing natural production. The therapy treats symptoms but can make the underlying testicular shrinkage worse over time.
What did he get wrong about treatment?
The creator implies testosterone therapy will reverse genital shrinkage, but the evidence doesn't support this for adult men. TRT can improve libido and erectile function but won't change penis size.
For true testicular atrophy from hypogonadism, TRT actually makes the problem worse by shutting down the hypothalamic-pituitary-gonadal axis. Clomiphene citrate or hCG are better options for preserving testicular size.
The "natty" hashtag is also misleading if he's promoting testosterone therapy. Taking exogenous testosterone is literally the opposite of natural hormone optimization.
What should young men actually know?
Penis size anxiety is common but rarely reflects actual medical problems. True micropenis affects only 0.015% of men, according to Mondaini et al. in BJU International (2002).
If you're genuinely concerned about delayed puberty or small testicles, see an endocrinologist. They can measure actual testosterone levels and testicular volume rather than relying on TikTok symptoms.
Most importantly, don't start testosterone therapy based on size insecurity. The side effects include permanent testicular shrinkage, fertility problems, and cardiovascular risks that aren't worth it for cosmetic concerns.
