Direct answer: The U.S. Ozempic pen containing 2 mg in 3 mL has labeled settings of 0.25 mg and 0.5 mg. For a 0.25 mg setting, the dose counter displays 0.25. The Instructions for Use say not to set a dose by counting clicks.
Do not confuse the pen total with the dose
The 2 mg printed as total contents describes the whole pen. It is not a 2 mg injection setting on this presentation. The other U.S. pen presentations have different total contents and labeled doses: 4 mg/3 mL for 1 mg doses and 8 mg/3 mL for 2 mg doses.
Where the setting comes from
The 0.25 mg and 0.5 mg settings are shown for the 2 mg/3 mL pen in the U.S. prescribing information. Click counts are not a manufacturer-provided conversion, so a chart that assigns a number of clicks to 0.25 mg is not supported by those instructions.
