You can, but you'll achieve sub-maximal weight loss. Some patients with gastroparesis or severe medication sensitivity use 0.25-0.5 mg semaglutide long-term for appetite suppression without titrating higher. This is off-label use at a sub-therapeutic dose. Discuss with your provider whether this approach makes sense for your goals.
How do I know if I'm a candidate for microdosing?
You're a candidate if you have prior GLP-1 intolerance, gastroparesis, a history of severe medication side effects, or strong preference for slower titration after informed consent discussion. If you're GLP-1-naive with no motility issues and normal medication tolerance, starting at the label dose is more efficient.
What's the difference between microdosing and low-dose maintenance?
Microdosing is a titration strategy (starting low with the intent to escalate). Low-dose maintenance is staying at a sub-therapeutic dose indefinitely because higher doses are intolerable or because the patient is satisfied with modest weight loss. Both use the same dose range, but the intent and endpoint differ.
Does insurance cover microdose prescriptions?
Brand-name GLP-1s are only covered at FDA-approved doses. Compounded GLP-1s are not covered by insurance regardless of dose. Patients pay out-of-pocket, typically $200-400 per month depending on the pharmacy and dose.
Can I switch from a microdose to a standard dose mid-treatment?
Yes. If you start at 0.125 mg semaglutide and tolerate it well, you can escalate to 0.25 mg at week 4, then follow the standard Wegovy schedule from there. The microdose start doesn't lock you into a slower track; it just delays the first step.
