Methodology
- What is counted: the all-in cash price a new U.S. self-pay patient pays for a 28-day supply of compounded tirzepatide, month to month, with no promo code. All-in means the medication plus any required membership or program fee, any mandatory consult or new-patient fee (amortized across months 1 to 3), shipping and supplies. Lab work is noted separately when a provider requires it.
- Two reference points per provider. Month 1 is the first 28 days at the lowest dose on the label schedule, 2.5 mg per week. Renewal is the fourth 28 days at 5 mg per week with any introductory price expired. Providers that price by dose use the tier that contains the reference dose; flat-price providers show the same figure in both columns; providers that publish only a starting price get a month 1 figure and a blank renewal.
- Prepaid terms are never shown as a monthly price. They appear in their own column with the term that unlocks them, and billing cycles that are not 28 days are converted on days (for example $499 per 77 days is $181.45 per 28 days).
- How prices are verified: on the verification date FormBlends fetches each provider's own public pricing page (the URL in the Source column) with a standard browser user agent and confirms that the figures in the row appear on that page. The date is recorded per row. Providers whose page could not be fetched, whose price sits behind a quiz or bot wall, or whose figures exist only in third-party reports are excluded from this edition and listed below with the reason. Nothing is estimated.
- FormBlends is a competitor in this market. Its figures come only from its published price list, the same module the checkout charges from, and it is treated under identical rules: not ranked, not highlighted, and its position in the summary is stated as a plain fact.
- A blank cell means the provider's page does not establish the figure. It never means zero. Compounded tirzepatide is not FDA approved, and a lower price is not a quality signal; this index does not evaluate pharmacies, prescribers or outcomes.

- Mochi Health: The membership fee ($79 per month) is on the FAQ page, but the page now says medication pricing varies by pharmacy and the homepage shows a struck-through price with a promotional figure, so an all-in month-to-month price could not be established on the verification date.
- Ivim Health: The compounded tirzepatide page shows only 'starting at $133 per month'; the required membership fee is not published in server-rendered HTML, so the all-in price could not be established.
- MEDVi: Pricing is behind an intake quiz; no public price on the provider's own pages.
- TrimRx: The pricing page is rendered by JavaScript and its server HTML carries no tirzepatide-specific price.
