How the wall works

Every card here was submitted by someone who checked the opt-in box on the Gave Back page. The wall is not seeded with fake stories or AI-written quotes. If the gallery is empty, it is because nobody has opted in yet. That is a promise, not a placeholder.

Each card shows a first name, a month number, and three things. Nothing else.

About the wall

How do submissions end up on the wall?

Only by explicit opt-in. When someone creates a card on the Gave Back page, a separate checkbox controls whether the card appears here. Cards default to private. Nothing shows up without that deliberate yes.

What information is shown with each card?

A first name (or nothing, if the user leaves it blank), a month number, and the three things they listed. Nothing else. No last names, no location, no dose, no weight, no photos.

Can I be removed from the wall?

Yes, any time. Email hello@formblends.com with the first name and approximate date. We remove the entry within one business day, no questions asked. Your story is yours.

Why are there not more cards yet?

Because we refuse to seed the wall with fake data. If the count looks small, it is small. Every card is from a real person who opted in. That is a feature, not a gap.

Is this content medically reviewed?

The wall presents patient-reported reflections, not medical claims. The surrounding page content (and the research context on the main Gave Back page) is reviewed by the FormBlends Medical Review Team. Individual cards are user-submitted and unedited.

Can I quote these cards elsewhere?

You can quote a card, but please do not attach a full name or contact information. Keep it to the first name shown. Respecting the anonymity is the whole point of the wall.
