What the Sermorelin dose evidence actually is
The only label (Geref, discontinued) covered children at 30 mcg per kg daily. Adult use at 200 to 500 mcg nightly is pharmacy practice, not a labelled dose.
The Geref label dosed 30 mcg per kilogram once daily at bedtime in children with growth hormone deficiency. Geref was discontinued and Drugs@FDA records that it was not withdrawn for safety or effectiveness reasons.
Adult sermorelin dosing of 200 to 500 mcg nightly, often five nights on and two off, is a compounding-pharmacy convention. It is widely used and it is not a label. Enter the dose your pharmacy label or prescriber gave you.
Sermorelin units at each water volume
The table uses a 5 mg vial. Each cell is the draw on a U-100 insulin syringe for that dose at that water volume. Cells above 100 units do not fit a 1 mL syringe; cells below 10 units sit in the part of a 1 mL barrel where measurement error rises, and a 0.5 mL or 0.3 mL syringe reads them better.

Worked example: 5 mg with 2 mL is 2.5 mg per mL, so 300 mcg (300 mcg, inside the range compounding pharmacies use for adults) is 12 units, or 0.12 mL.
Mixing Sermorelin
A 5 mg vial with 2 mL gives 2.5 mg per mL, so 200 mcg is 8 units, 300 mcg is 12 units and 500 mcg is 20 units. The 200 mcg draw lands between the 2-unit marks on a 1 mL syringe, which is where a 0.5 mL syringe with 1-unit marks earns its keep.
A 5 mg vial with 1 mL doubles the concentration and halves every draw. That puts 200 mcg at 4 units, which the accuracy research says is where measurement error climbs past 10 percent. More water is the better choice for nightly microgram doses.
Storing Sermorelin after mixing
Refrigerate after mixing. A 5 mg vial at 300 mcg nightly is about 16 doses, comfortably inside 28 days. A 10 mg vial at 200 mcg nightly is 50 doses, which is past the multiple-dose ceiling; the calculator flags it.

Two millilitres gives 2.5 mg per mL, which puts 200 to 500 mcg doses between 8 and 20 units. That is the range where insulin syringes are most accurate.
How many units is 300 mcg of sermorelin?
Twelve units at 2.5 mg per mL, 6 units at 5 mg per mL, 24 units at 1.25 mg per mL.
Is there an FDA-approved adult sermorelin dose?
No. The only label covered children and the product was discontinued. Adult ranges come from compounding practice.
Does sermorelin need to be taken at night?
The pediatric label specified bedtime dosing, and compounding protocols kept that convention because growth hormone release peaks in early sleep. The calculator does not change with timing; your prescriber decides that.
