What the Tesamorelin dose evidence actually is
Label: Egrifta SV 1.4 mg daily (2 mg vial mixed with 0.5 mL), Egrifta WR 1.28 mg daily (11.6 mg vial mixed with 1.3 mL for seven doses). Original Egrifta was 2 mg daily.
Egrifta is approved to reduce excess abdominal fat in adults with HIV-associated lipodystrophy. Egrifta SV is a 2 mg vial reconstituted with 0.5 mL of sterile water and given as 1.4 mg (0.35 mL) once daily; the vial is single-use because sterile water has no preservative. Egrifta WR is an 11.6 mg vial reconstituted with 1.3 mL of bacteriostatic water and gives seven daily doses of 1.28 mg (0.16 mL). The original Egrifta was 2 mg once daily from two 1 mg vials.
Tesamorelin's half-life is minutes, which is why it is dosed daily and why the timing people argue about matters less than consistency. Off-label anti-aging protocols use 1 to 2 mg daily, which mirrors the label range.
Tesamorelin units at each water volume
The table uses a 5 mg vial. Each cell is the draw on a U-100 insulin syringe for that dose at that water volume. Cells above 100 units do not fit a 1 mL syringe; cells below 10 units sit in the part of a 1 mL barrel where measurement error rises, and a 0.5 mL or 0.3 mL syringe reads them better.

Worked example: 5 mg with 1 mL is 5 mg per mL, so 1.4 mg (1.4 mg, the Egrifta SV labelled daily dose) is 28 units, or 0.28 mL.
Mixing Tesamorelin
A 5 mg vial with 1 mL gives 5 mg per mL, so 1.4 mg is 28 units and 2 mg is 40 units. With 2 mL the same doses are 56 and 80 units. Because it is used daily, most people prefer the stronger mix and the smaller draw.
The Egrifta WR label is the best real-world guide to multi-dose tesamorelin: about 8.9 mg per mL, seven doses, bacteriostatic water, refrigerated between doses.
Storing Tesamorelin after mixing
Refrigerate after mixing with bacteriostatic water and use within 28 days of first puncture. A 5 mg vial at 1.4 mg daily is three full doses, so it never approaches that limit. A 20 mg vial at 1.4 mg daily lasts two weeks.

One millilitre gives 5 mg per mL and puts the 1.4 mg labelled dose at 28 units. Two millilitres gives 2.5 mg per mL and 56 units. Either fits a 1 mL syringe; the 1 mL mix also fits a 0.5 mL syringe.
How many units is 2 mg of tesamorelin?
Forty units at 5 mg per mL, 80 units at 2.5 mg per mL, 20 units at 10 mg per mL.
Why does Egrifta SV use sterile water instead of bacteriostatic water?
Because it is a single-use vial. Sterile water has no preservative, so anything left after the dose is discarded. Egrifta WR, the multi-dose version, uses bacteriostatic water for that reason.
What is the difference between 1.4 mg and 2 mg tesamorelin?
They are two labelled products. The original Egrifta dose was 2 mg daily. Egrifta SV delivers 1.4 mg daily from a more concentrated formulation. Both are label doses for the approved indication.
