How to Use This Template
Weight loss is the headline, but GLP-1 therapy success is about much more than the number on the scale. This worksheet uses the SMART goal framework (Specific, Measurable, Achievable, Relevant, Time-bound) to help you set and track goals across four categories. Weight Goals: target weight, interim milestones, realistic timeline based on 1-2 lbs per week. Measurement Goals: waist, hips, and other circumference targets. Health Marker Goals: A1C target, blood pressure target, cholesterol targets. Non-Scale Victory Goals: energy level improvement, sleep quality, clothing size changes, exercise milestones, medication reduction (with provider approval). Each category has space for a 1-month, 3-month, 6-month, and 12-month milestone. The bottom section includes a monthly check-in grid where you rate your progress and adjust goals as needed.
