FormBlends Template Context
Reviewed May 14, 2026The strongest way to read Injection Site Rotation Map template is to look for what changes the next step. For dosing and safety, that means checking whether the page is explaining evidence, eligibility, cost, safety, provider fit, or day-to-day use. The goal is not more words on the page. It is a clearer path from a broad question to a responsible medical conversation.
- Fill it out with dates, doses, symptoms, and provider instructions instead of memory alone.
- Keep a copy for refill visits, lab reviews, insurance appeals, or side-effect conversations.
- Do not use a printable to change dose or medication without clinical guidance.

How to Use This Template
Rotating injection sites prevents lipodystrophy (changes in fat tissue at the injection site), reduces pain, and improves medication absorption consistency. This map shows front and back body diagrams with six clearly numbered injection zones: left abdomen, right abdomen, left thigh, right thigh, left upper arm, and right upper arm. A rotation schedule shows which site to use each week on a 6-week cycle, and a tracking grid lets you mark exactly where in each zone you injected (at least 1 inch from the previous spot within the same zone). The sheet also includes tips on which sites tend to cause the least pain and which to avoid (within 2 inches of the navel, areas with scars or bruises, waistband area).
