Why the first-month number can mislead
An introductory offer may apply only to starter doses and a limited number of fills. Your total 12-month cost depends on dose progression, whether you remain eligible, and whether the program changes. Ask for the expected price at the next two doses before starting.

The monthly price hides how the first year adds up. These totals use Novo Nordisk's self-pay prices as published in June 2026 and confirmed on September 4, 2026, and assume no insurance, no card, and no missed fills.

The retail figure is GoodRx's listed cash price on September 4, 2026; SingleCare shows $1,807 for thirty 1.5 mg tablets. Nobody should be paying it, and if a pharmacy quotes it, the offer was not applied.
Wegovy without insurance at CVS, Walgreens, Walmart and Costco
The pharmacy does not change the price. On September 4, 2026, GoodRx listed the identical $149.00 Wegovy pill price at CVS, Walgreens, Walmart, Costco, Sam's Club, Target and Capsule, because all of them dispense at Novo's self-pay price. Where they differ is stock, delivery and whether the technician processes the fill correctly as a cash claim with the offer. If a quote comes back near $1,600, ask them to rerun it as self-pay with the Novo offer rather than through insurance.
When your insurance does not count: Medicare, Medicaid and the $50 Bridge
Two groups technically have insurance and still pay cash for Wegovy. Medicare Part D excludes weight-loss drugs, and Novo's self-pay offer excludes government beneficiaries. Since July 1, 2026 the Medicare GLP-1 Bridge fills that gap: Wegovy pens and pills, and the Zepbound KwikPen, for no more than $50 a month with prior authorization, through December 31, 2027, outside the Part D deductible and the $2,100 cap.
Medicaid is state by state. As of January 2026, 13 state Medicaid programs covered GLP-1s for weight loss, according to SingleCare's tally, and several more added coverage or ordered cost studies during 2026. If your state is not one of them, the self-pay prices above apply.
Free Wegovy: the Novo Nordisk patient assistance program
Novo Nordisk's patient assistance program supplies Wegovy at no cost to U.S. residents with household income at or below 400 percent of the federal poverty level who have no coverage for it. Medicare Part D patients under 150 percent of the poverty level must first be denied Extra Help. Applications for the following year open after October 15, 2026. The program is the only route to a $0 Wegovy for uninsured patients, and it is worth the paperwork before paying cash.
How compounded semaglutide compares in September 2026
The FormBlends Compounded GLP-1 Price Index, which observes provider pricing pages weekly, put the median month-to-month all-in price of compounded semaglutide at $197 for the first month (14 providers, range $88 to $299) and $224 at the 1 mg step on September 3, 2026. FormBlends' own entry price is $99. Against a $199 then $349 Wegovy pen, the gap is real but far smaller than the $1,000-plus comparisons most articles still make. Compounded semaglutide is not FDA approved, and FDA's page on the subject, current as of September 1, 2026, reports 990 adverse-event reports for it, so the choice is about more than price.
The two dates that change these prices
Novo's offer terms carry two deadlines. The 4 mg Wegovy pill's $149 price applied to fills through August 31, 2026 and moved to $199 after that. The $199 introductory pen price applies to fills redeemed by December 31, 2026, after which new patients start at $349. GoodRx's card box still showed the older August 31 expiry for the injection on September 4, 2026; the NovoCare terms page is the one to trust.
