Extended-release (XR or ER), brand-name Glucophage XR or Fortamet: taken once daily, usually with dinner. The same total daily dose distributed over 24 hours. Published data shows 30 to 40% lower GI side effect rates than IR (Blonde et al., Current Medical Research and Opinion 2004). Slightly more expensive but available as a generic since 2009.
For weight-loss use, most providers prefer XR. The once-daily dosing improves adherence, and the smoother release flattens the GI side-effect curve. The total daily dose to target is the same: 1,500 to 2,000 mg.
One caveat. If you cut or crush XR tablets, you destroy the extended-release coating and convert it to immediate-release with all the GI side effects you were trying to avoid. Swallow XR tablets whole.
