What does Kaiser require if your plan does cover GLP-1s?
When a Kaiser plan does include the weight loss benefit, expect prior authorization and clinical criteria similar to other insurers:
- A qualifying BMI, generally 30 or higher, or 27 or higher with a weight related condition such as diabetes, high blood pressure, high cholesterol, or sleep apnea.
- Documentation that you tried diet and exercise, often for three to six months, recorded in your Kaiser chart.
- Enrollment in a Kaiser weight management program in many cases.
- Reauthorization later that shows you lost a meaningful amount of weight, commonly at least 5 percent.
