Levocarnitine is the medical name for L-carnitine. It is a small molecule that cells use to move fatty acids into mitochondria, the parts of the cell that burn fuel. Your body makes it, and you also get it from meat and dairy. In a compounded product, the pharmacy dissolves tirzepatide and levocarnitine together so both come out in one weekly injection.

Does carnitine change the tirzepatide dose?
No. The dose is the tirzepatide dose your prescriber wrote. The label model is 2.5 mg once weekly for 4 weeks, then increases of 2.5 mg after at least 4 weeks on the current dose, up to 15 mg. The carnitine is simply present in the liquid at whatever amount the pharmacy chose. The number that matters for drawing your dose is the tirzepatide concentration on the vial.
